Client-Driven Program
Clients, in collaboration with clinical staff, develop an individual client service plan (CSP) that is unique to that individual and is driven by the client’s self-identified goals. In order to engage clients in progressing through their program on a daily basis we have developed a Weekly Responsibility Sheet, that allows clients to earn points in a variety of program areas such as:
- Therapeutic and process groups
- Recreational activities
- Independent life skills
- Health & Self-care activities
- And, overall program participation
Palm View Retreat Pass Levels
Black Out: First 14 days Resident will be in black out. Residents may go on Staff Outings to include: Store runs, recreational activities, and doctor appointments. Resident must earn a minimum of 3570 points per week and have no
behavioral issues or rules infractions to move forward.
Level 1 Buddy Pass: 3570 – 5049 Points
For the first two weeks of buddy passes, residents can take one hour passes into the community with a buddy twice a day. Resident must earn a minimum of 1305 points to maintain this level. After first week resident must earn a minimum of 3570 points and have no behavioral issues or rules infractions to move on to Level 2.
Level 2 Buddy Pass: 5050 – 5999 Points
Residents can take two hours twice a day to go out in the community with another ‘buddy’. Resident must earn a minimum of 5050 points to maintain at this level. After first week resident must earn a minimum of 6000 points and have no behavioral issues or rules infractions to move on to Level 3 Solo Passes.
Level 3 Solo Passes: 6000 – 6699 Points
Residents can take two hours twice a day to go out in the community by him/herself. Resident must earn a minimum of 6000 points to maintain at this level. After first week resident must earn a minimum of 6700 points and have no behavioral issues or rules infractions to move on to Level 4 Solo Passes.
Level 4 Solo Passes: 6700+ Points
Residents can take 4 hours once a day, or 2 hours twice a day to go out in the community by him/herself. Resident must earn a minimum of 6700 points for 8 consecutive weeks and have no behavioral issues or rules infractions to advance to Level 5 “PATHFINDERS.”
Level 5 Pathfinders: 6700 POINTS and…
Residents who attain this level must not be on behavioral contract, have no rule I infractions, attend 2 weekly WRAP groups, pass weekly room inspections, and attend any assigned groups to remain a PATHFINDER. Residents are eligible for 5 hours a day in the community, all together or broken up into
one-hour increments as the resident chooses. Resident is also eligible for special privileges, outings,
and events. SPECIAL PRIVILEGES INCLUDE: Use of TV until 11:30pm; an extra 11pm cigarette break; Transportation Training; bus passes; and attending special outings and events.
